Superbilling
Commercial Insurance Reimbursement
Use the billing codes listed by service when submitting for out-of-network reimbursement. A superbill will be provided to you. Please note that it is likely that only a portion will be reimbursed by your insurance company.
Back to Fee ScheduleBefore you submit a superbill:
Please check with your insurance provider before submitting a superbill. Check frequency limits for claims and utilization rates.
Paying with an HSA or FSA card:
HSA and FSA cards are accepted through Jane Payments, but approval is decided by your card issuer and is not guaranteed. Please confirm with your plan administrator which services are eligible medical expenses. Court, forensic, and legal services and clinical supervision are often not eligible. A backup credit or debit card is required on file in case an HSA/FSA card is declined. If a card is declined, contact your card issuer directly for the reason.
